Dr. Geeteshwar Mangat is a board-certified internal medicine physician and the founder and President of Synchrony Medical Group. He and his wife, Dr. Amolika Mangat, are raising their three children in Bakersfield, the community they have served for nearly two decades.<\/p>

Over his career caring for hospitalized patients, Dr. Mangat saw how often a patient\u2019s recovery depended on what happened after discharge. He founded Synchrony to close that gap and to create an environment of healing that follows patients wherever they are. Today he cares for patients across the full continuum of care: in inpatient rehabilitation, in skilled nursing facilities, and in the clinic. He serves as Rehabilitation Medical Director at Central California Rehabilitation Hospital, where he oversees the medical care of patients recovering from stroke, surgery, injury, and serious illness.<\/p>

Dr. Mangat has built a team that shares this vision. He works alongside physiatrist Dr. Maria Mercedes Melendez at the rehabilitation hospital, and with nurse practitioners who round daily in rehab and skilled nursing settings and provide after-hours coverage. When patients are ready to go home, Synchrony\u2019s clinic continues their care with providers who already know their story.<\/p>

Dr. Mangat earned his medical degree from Dayanand Medical and completed her internal medicine residency at SUNY Downstate Health Sciences University in Brooklyn, New York. He has contributed to published research on sepsis, COPD, and COVID-19. He speaks English, Hindi, and Punjabi.<\/p> <\/div> <\/div> <\/div>

Dr. Amolika Mangat<\/h2>

Internal Medicine, Obesity Medicine & Menopause Care<\/em><\/p>

Dr. Amolika Mangat is a physician, a mother of three, and the co-founder of Synchrony Medical Group with her husband, Dr. Geeteshwar Mangat. She is double board-certified in Internal Medicine and Obesity Medicine and is a Menopause Society Certified Practitioner (MSCP), a combination of expertise that is rare in the Kern County area.<\/p>

\"Menopause<\/a><\/figure>

Dr. Mangat provides primary and preventive care for adults, with a special focus on weight management, metabolic health, and women\u2019s health. Her obesity medicine practice treats weight as a medical condition, not a matter of willpower. She builds personalized plans that may combine nutrition, physical activity, behavior change, and, when appropriate, medication, and she tracks progress with tools such as in-office body composition testing. Her recent certification from The Menopause Society<\/a> strengthens Synchrony\u2019s support for women in perimenopause and menopause, including evidence-based guidance on hormone replacement therapy (HRT) and non-hormonal options. In 2025 she helped open Synchrony\u2019s Tehachapi clinic, bringing this specialized care to a community where it had been hard to find.<\/p>

Dr. Mangat believes caring for a patient means understanding the whole person, not just a list of symptoms. She takes time to learn each patient\u2019s story and works with them on lasting changes that support mind, body, and overall wellness. She has also helped train the next generation of clinicians, including Synchrony nurse practitioner Meghan Escoto, who completed a clinical rotation with her as a student.<\/p>

Maria Mercedes Melendez, M.D.<\/h2>


Physical Medicine & Rehabilitation | Central California Rehabilitation Hospital<\/em><\/em><\/p>

Dr. Maria Mercedes Melendez is a physical medicine and rehabilitation physician, also called a physiatrist, who is also fellowship-trained in hospice and palliative medicine. She cares for patients at Central California Rehabilitation Hospital in Bakersfield, where she works closely with Dr. Geeteshwar Mangat and Dr. Amolika Mangat so that each patient\u2019s rehabilitation and medical care are fully coordinated.<\/p>

Physiatrists focus on restoring function and quality of life, not just treating a diagnosis. Dr. Melendez directs rehabilitation care for patients recovering from stroke, brain and spinal cord injury, joint replacement, and other illnesses and injuries that affect mobility and daily living. She works with physical, occupational, and speech therapists to set clear recovery goals and help each patient regain as much strength and independence as possible.<\/p>

Dr. Melendez brings more than 15 years of practice experience across the full range of post-acute care. In Washington State she led inpatient rehabilitation programs, consulted in long-term acute care hospitals and skilled nursing facilities, and served as a hospice medical director. Earlier, at Mercy Hospital in St. Louis, she started and led a palliative care clinic. Her palliative care training gives her particular skill in managing pain and symptoms and in guiding patients and families through difficult decisions with compassion.
Her partnership with Synchrony Medical Group gives patients a team that knows them well. While Dr. Melendez directs their rehabilitation, Dr. G. Mangat and Dr. A. Mangat manage their medical needs. When patients go home, Synchrony\u2019s clinic is ready to continue their care.<\/p>

Dr. Melendez earned her medical degree from the Medical College of Ohio in Toledo and completed a surgical internship there. She then completed her residency in Physical Medicine and Rehabilitation and a fellowship in Hospice and Palliative Medicine at the University of Texas Southwestern Medical Center in Dallas. She is board-certified by the American Board of Physical Medicine and Rehabilitation and speaks English and Spanish fluently.<\/p>

With extensive training in aesthetic procedures, skin health management, biopsies, benign lesion removal, and advanced dermatologic therapies, Jessica is committed to delivering results that look natural, healthy, and uniquely tailored to each individual. Jessica earned her Master of Science in Nursing and has built her career in a variety of high-acuity settings including urgent care, postanesthesia care, telemetry, and hemodialysis. Her background as a charge nurse and team leader further enriches her collaborative, steady, and patient-first approach.<\/p>

Meghan Escoto is a board-certified nurse practitioner with more than a decade of experience in health care, from the hospital ICU to the family exam room. She is a familiar face in Tehachapi, where she has cared for local patients since 2021, and she brings that neighborly warmth to every visit at Synchrony Medical Group.
Meghan began her career as a critical care nurse at San Joaquin Community Hospital and later worked as an endoscopy nurse. Those years at the bedside gave her a deep understanding of how serious illness unfolds and how much good early, attentive primary care can do. She went on to practice as a nurse practitioner in primary care and in neurology, where she treated patients with migraines and other neurological conditions. Her connection to Synchrony goes back to her training: one of her first clinical rotations as a nurse practitioner student was with Dr. Amolika Mangat.
Today Meghan provides primary and preventive care for teens and adults. That includes annual and senior wellness exams, management of chronic conditions, and treatment of acute illness. She believes good health includes emotional and mental well-being as well as physical, and she helps patients navigate the health care system so they get the right care at the right time.
Meghan earned her Bachelor of Science in Nursing from Pacific Union College and her Master of Science in Nursing as an Adult-Gerontology Acute Care Nurse Practitioner from Walden University. She is board-certified through the American Nurses Credentialing Center (ANCC).<\/p>

Outside the office, Meghan loves to travel with her husband, Ben, and spending time with their fur baby, Zin.<\/p> <\/div> <\/div> <\/div>

Kami Dhindsa has cared for patients in Bakersfield for nearly 30 years. She has worked at almost every point in a patient\u2019s journey: in skilled nursing and hospice care, at the hospital bedside, and in helping patients and families plan their care after discharge. That deep experience makes her a natural fit for Synchrony Medical Group\u2019s commitment to caring for patients beyond the hospital.<\/p>

Kami began her career in 1997 as a licensed vocational nurse, caring for hospice patients and residents of local skilled nursing communities. After becoming a registered nurse, she spent 14 years at Mercy Hospital in Bakersfield, working in medical-surgical, orthopedic, intensive care, and emergency units. She then moved into case management at Mercy, where she coordinated care for complex patients, advocated for them as they made treatment decisions, and connected families with the services they needed after leaving the hospital.
At Synchrony, Kami rounds daily on patients in skilled nursing and inpatient settings. She also provides after-hours coverage, so patients and the facility teams caring for them can reach a Synchrony provider when questions come up overnight or on weekends. Her years in skilled nursing, hospital, and case management roles mean she understands what patients need to recover safely, and what families need in order to feel supported.<\/p>

Kami earned her Bachelor and Master of Science in Nursing from Grand Canyon University and completed her Family Nurse Practitioner training at United States University. She speaks English, Hindi, and Punjabi.<\/p> <\/div> <\/div> <\/div>

Prabhjot Grewal, NP<\/h2>

Nurse Practitioner | Synchrony Medical Group<\/em><\/p>

Prabhjot Grewal has spent nearly a decade caring for patients in Bakersfield, from transitional care to the intensive care unit. She now brings that experience to Synchrony Medical Group as a nurse practitioner, following patients from the rehabilitation hospital back to the clinic.<\/p>

Prabhjot began her career as a licensed vocational nurse at Kern River Transitional Care, where she cared for patients recovering after a hospital stay and earned a reputation for excellent patient satisfaction. She went on to become a registered nurse in the cardiac, medical, and neurological intensive care units at Bakersfield Memorial Hospital. There she cared for critically ill patients after heart surgery, stroke, and serious infection, and served as a resource nurse and preceptor for other nurses.<\/p>

Today most of Prabhjot\u2019s week is spent rounding at the rehabilitation hospital alongside Dr. Geeteshwar Mangat, caring for patients recovering from stroke, surgery, injury, and serious illness. She also sees patients in Synchrony\u2019s clinic, often continuing care for the same people she met during their hospital stay. Because she has worked at every stage of recovery, she understands what patients and families face after discharge, and she helps make each transition feel like the next chapter of the same care.<\/p>

Prabhjot earned her Bachelor of Science in Nursing from Capella University, and her MSN, Family Nurse Practitioner from United States University. She speaks English, Punjabi, and Hindi.<\/p> <\/div> <\/div> <\/div> <\/div> <\/div>

Sarah Shelbourne<\/h2>

Chief Operating Officer & Chief Financial Officer | Synchrony Medical Group<\/em><\/p>

Sarah Shelbourne has spent three decades building and leading health care organizations in Kern County. As COO and CFO of Synchrony Medical Group, she oversees the practice\u2019s operations, finances, and growth, so the physicians and nurse practitioners can focus on what matters most: caring for patients.
Sarah\u2019s career has covered nearly every part of the local health care landscape. She began in physician practice management with Catholic Healthcare West, helping form and administer a 20-provider primary care medical group. She then spent 16 years with Optimal Health Services, rising from Home Health Operations Director to Controller, CFO, and ultimately CEO of what was then the largest privately held hospice and home health provider in California. Most recently, as Physician and Ambulatory Services Executive for Adventist Health in Kern County, she led 17 medical offices and rural health clinics with 75 providers. During that time she grew annual patient visits from 100,000 to 150,000, launched a new urology service line, and expanded access to primary and specialty care in rural communities.<\/p>

That experience shapes her work at Synchrony. Sarah has seen firsthand how hospital, clinic, skilled nursing, home health, and hospice care fit together, and she shares the Mangats\u2019 commitment to coordinated care that follows patients beyond the hospital. She focuses on making care easier to reach, improving the patient experience, and building a practice that can keep growing to serve the community.<\/p>

Sarah earned her Bachelor of Science in Business Administration from California State University, Bakersfield. She is the proud mother of two, Alex and Riley, and enjoys travel, wine tasting and spending time with family and friends.<\/p> <\/div> <\/div> <\/div>

Mariah Sanchez is our Office Manager and the organizational anchor of our clinic. With nearly a decade of experience in urgent care and multisite medical practices, Mariah brings exceptional leadership, operational expertise, and a deep commitment to patient-centered care.<\/p>

Mariah began her career as a Certified Medical Assistant, gaining hands-on experience in both clinical and administrative settings. Her strong clinical foundation\u2014combined with her natural leadership skills\u2014led her to advance into Clinical Manager and Clinical Operations Supervisor roles, where she successfully oversaw daily operations, staff development, and compliance across multiple locations.<\/p>

As Office Manager, Mariah ensures that every aspect of the clinic runs smoothly behind the scenes. She coordinates front and back office workflows, supports providers and clinical staff, and continuously works to improve efficiency, communication, and patient experience. Her expertise includes staff training and scheduling, insurance verification and authorizations, EMR management, inventory oversight, and quality assurance.<\/p>

Mariah is known for her calm, solution-oriented approach, even in fast-paced or high-volume environments. She plays a key role in creating a welcoming and organized experience for patients\u2014from the moment they contact our office through the completion of their care.
Above all, Mariah is passionate about supporting both patients and staff.
